What do I bring as a new patient?
Please bring your insurance card upon your first visit. You much also fill out our New Patient Registration, Medical History, Financial Policy, Privacy Policy and Authorization forms. To speed up the new patient process you can complete the forms ahead of time and bring them to your appointment. You can get to our forms by going to the Patient Forms page.
What types of payment do you accept?
We accept payment by cash, check, VISA, Mastercard, American Express and CareCredit.
What procedures do you refer out?
Endodontic and orthodontic procedures, as well as some oral surgeries. Please call us for a consultation at 910-791-2401.
What are your office hours?
Our offices are open Monday – Thursday from 8am to 5pm.
